Eligibility Requirements

  • Age 65 or older

    You are eligible for Medicare when you turn 65

  • Under 65 with disabilities

    You might be eligible if you have certain disabilities

  • Any age with End-Stage Renal Disease (ESRD)

    Permanent kidney failure requiring dialysis or transplant
